Mary’s journey to healing
Mary was struggling. Arthritis, chronic pain, and depression made everyday life hard and threatened her independence.
Then she joined the Medically Vulnerable Program, a collaboration between Fourth Street Clinic in Salt Lake City, Utah, and the Intermountain Community Care Foundation. The program provided Mary with safe housing, coordinated medical care, and emotional support. Over nine months, her pain eased, her health improved, and she rediscovered her confidence to live independently.
Across the communities we serve, many individuals face similar barriers—health conditions complicated by housing instability, lack of preventive care, or limited support systems. Intermountain Health’s collaborative approach addresses these challenges, directly transforming lives long before someone walks through our health system’s doors. This is the daily work of Community Health and many others who help make care accessible and responsive to what our communities need most.
Prevention in action
Community collaboration takes many forms, from clinical programs to simple acts of prevention that keep people safe and connected.
At a recent bike safety event in Henderson, Nevada, Intermountain Community Health partnered with Roseman University’s Genesis Program, Clark County School District’s Safe Routes to School, Club Ride – RTC, and Smalley Elementary School. Together, the group distributed 100 helmets and safety cards to students and families, turning injury prevention into fun and games.
“You could see the impact in every smile,” said Jennifer Grube from Clark County School District.
So far in 2025, Intermountain has distributed more than 370 bike helmets to underserved youth in southern Nevada.
Building healthier tomorrows
As a nonprofit, we prioritize community wellness and prevention. Guided by Community Health Needs Assessments, Intermountain uses data and local insights to shape initiatives that support people’s well-being early and throughout life.
When prevention isn’t enough, Intermountain invests in specialty care to ensure people still receive the help they need. Together, these efforts form a full spectrum of support that strengthens both individuals and communities.
“When community organizations, caregivers, and partners come together, we can make health possible in every place we serve. Intermountain is building a broader system to support this work—ensuring that collaboration is not just a philosophy, but a foundation for how we deliver health,” said Lisa Nichols, Vice President of Community Health.
