You can use Intermountain’s My Health+ digital health app to access confirmation that you received your COVID-19 vaccine. You’ll see which vaccine you received, which date(s), and where you were vaccinated. Here’s how to access your COVID-19 vaccine record:
- Download the My Health+ app from the App Store or Google Play, or visit My Health+ on the web at Intermountain.com/app.
- Login using your existing My Health+ username and password. If you don’t have one, create a My Health+ account online or in the app.
- To access the records of those you care for, such as children or dependents under 18, follow the instructions provided at IntermountainHealthcare.org/mmrecords.
- Navigate to your immunization record. Select “Health Record,” then “Medical History,” then “Immunizations.” Download and print your vaccine record, or you can email them.
