Your care team will ask you to complete a short questionnaire before and after surgery or during treatment. You can do this before your clinic or hospital visit, online, through a text message link, or during your clinic visit. It may ask about your pain, movement, and daily activities. Your answers help your care team see how you are doing and plan your care.
