Yes, we require a referral from your primary care provider. The referral needs to include the most recent chart note and pertinent labs. We have a nursing team that triages each referral that we receive.

 

Some insurance plans also require a referral authorization. We recommend checking with your insurance company before scheduling or contacting our office and we can help guide you.

Please bring a photo ID, insurance card, a list of current medications, recent lab results (if available), and any relevant medical records. For diabetes visits, bringing glucose logs or device data is also very beneficial.
