Our multi-disciplinary team-based care is patient centered to address the unique needs of each patient. Our team will work with you to develop a plan of care based on your needs including a medication assessment, physical assessment and individual group therapy and planning for discharge. Medication is an important part of treatment for most patients and we ensure that medication is only provided to the right person with the right medication. support from family engagement and different therapy options such as recreational therapy from professionals such as licensed clinical social workers to ensure high-quality treatment
