    Dr. Wyatt Horsley has been a board-certified family physician since 2016. He has served as both an attending physician in his own clinic and in an academic setting training residents.

    Concierge Health is a premium healthcare membership-based program that provides enhanced access to primary care services for individuals and families. We offer a proactive and patient-centered approach to medical care, focusing on delivering individualized healthcare services and medical navigation.

    Our program provides more one-on-one interaction and time with your physician and care team and offers virtual visits for convenience. 

    You can expect several key benefits and features to enhance your healthcare experience.

    • Personalized, comprehensive care. We focus on individualized and patient-centered care. Your membership includes a personalized medical assessment to meet your healthcare needs.
    • Coordination of care. Our concierge health team takes a proactive approach to coordinating your healthcare. We work with a full range of specialists in the Intermountain network as well as providers from other healthcare companies, including health and wellness providers in the community.
    • Enhanced access and availability. We have a limited patient panel, so you can enjoy more extended and unhurried office visits. You can also reach out 24/7 for medical needs by contacting our on-call provider.
    • Wellness and prevention focus. We work with you to identify risk factors to prevent illness and promote well-being. You can expect to receive comprehensive primary care, health screenings, routine vaccinations, and personalized guidance on nutrition, exercise, and lifestyle choices.
    One of the many conveniences of our program is that a majority of your care can be managed virtually by video visits or phone/direct messaging. If an in-person visit is more appropriate, our clinic is located at Intermountain’s Park City Hospital. No matter how you communicate with your doctor, you will experience the premium care you can expect as a member.
    While our annual membership fee is not covered by insurance, it does grant you access to the enhanced wellness and comprehensive primary care services we offer. We do recommend you still carry insurance, and we will bill your insurance for covered services such as vaccinations, skin biopsies, etc. You may have the opportunity to use your health savings account (HSA) or flexible spending account (FSA) to cover your annual fee. Please check with the administrator of your account(s) to verify.
