What we treat

Many people with different illnesses, injuries or conditions can benefit from inpatient rehabilitation. Typically, patients who benefit from this level of care have difficulty in at least two of these areas: movement, caring for yourself, thinking, communication or swallowing.

We treat the following conditions:

  • Stroke
  • Brain injury
  • Spinal cord injury
  • Neurological conditions
  • Severe weakness due to medical problems
  • Major orthopedic or other traumatic injuries
  • Amputation
  • Other illnesses, injuries or conditions that impair your ability to return home from the hospital and require our services

A multi-specialty, team-based approach

Your skilled and caring team will meet weekly to discuss your care plan and your progress, and to adjust goals as necessary to ensure you are progressing at an appropriate speed. Depending on your individual care plan, your team may consist of:

  • A board-certified physician, specializing in physical medicine and rehabilitation.
  • Nurses specifically trained in rehabilitation
  • Physical therapists to help improve balance, coordination, mobility, and strength
  • Occupational therapists to address self-care and daily living activities
  • Speech language pathologists to improve cognitive deficits, communication, and swallowing
  • Social workers and care managers to coordinate care in the hospital, arrange care after discharge, and provide counseling as needed
  • Dietitians to ensure proper nutrition through your treatment program
  • Access to peer mentors to share their past lived experiences going through a similar challenge.
  • And others as needed including neuropsychology, rehab psychology, respiratory therapy, wound therapy, prosthetics, dialysis, and other specialists to ensure you have the care you need

What to expect during your stay

Frequently asked questions

During your stay, you will receive medical, nursing and therapy care every day. At least 5 days a week, you will receive more intensive therapy services – at least 3 hours – scheduled with rest breaks, if you need. On the other days, you will still receive therapy, but less intensively. Our services are provided every day, including weekends and holidays.

While on the unit, your rehabilitation plan may include a variety of treatments, services and locations, such as:

  • Individual therapy with one or, on occasions, two therapists at the same time
  • Therapy with one or more other patients to allow you to practice new skills and interact with others who have similar needs
  • Therapy and/or meetings with a family member or other representative of your choice to ensure that everyone is ready for your return home
  • Practice in areas that simulates your home, like a kitchen or bathroom
  • A visit to your own home, if you live near the hospital. This can be helpful for you to practice how you will get in or out of your home, bathroom, kitchen and other areas of your home
  • A visit to a local community business or site, such as a grocery store, bank or park to practice your new skills in the real world
  • Use of new and familiar technology resources such as high-tech rehabilitation equipment, computers, smart phones and other devices to help you when you return home

For most patients, your hospital physician and care team will recommend that you be evaluated for inpatient rehabilitation services.

One of our specialists – a rehab liaison – will visit with you and review your medical records. If you might benefit from our care, the liaison will discuss this with you, your family or representative, your current care team, the inpatient care team, and your insurance company, if appropriate. Some insurance companies require that we pre-authorize your stay to ensure that it meets certain criteria.

While many of our patients are referred while in the hospital, we also receive referrals for patients who are currently in a care facility or even at home.

Comfortable clothes work best for rehab. Please leave valuable items at home.

We suggest about four changes of clothing, including the following:

  • Glasses, hearing aids, dentures - anything you need to function on a day-to-day basis as those are the activities our therapists will be working on with you
  • Shorts or loose-fitting pants, T-shirts or blouses, tennis shoes, and a light jacket or sweater
  • Sleepwear if you prefer your own, or we can furnish hospital gown/pants
  • Any personal items that will make your stay more comfortable, such as photos, or a blanket.

 

We want family members, or others that you choose to represent you, to be a part of your care. We invite them, with your permission, to participate in therapy sessions and visit regularly so everyone can be prepared for your return home.

Sometimes, depending on distance or public health conditions, we are unable to have everyone come to the hospital. In these instances, we will work with you and your loved ones to find other ways to connect using the phone or video calls.
