Keeping patients closer to home

When a patient in a rural community needs to receive specialty care, the patient is often transferred to a facility hundreds of miles away from their families and communities – the support system they need most. Meanwhile, rural hospitals lose much-needed revenue, and communities face the very real possibility of losing their critical access hospital. Intermountain Telehealth is reversing this trend, keeping patients as close to home as possible. Allowing them to receive high-quality specialty care from Intermountain’s world-class experts no matter where they are.

Partnering with Intermountain Telehealth

We strive to help our partners improve access to care, lower costs, retain revenue and improve patient experience. Some of the ways we’ve been able to help our partners include:

Intermountain Health partners gain access to our internationally recognized care process models, build collaborative relationships with and learn from best-in-class specialists, and gain the benefits of partnering with a large healthcare system while maintaining independence.

Partners also receive consultations from Intermountain Health Clinical Coordination teams, including intensivists and hospitalists. This team will consult with the onsite clinicians to determine if we can treat the patient at your facility, or if a transfer is needed. If the patient needs to be transferred, our team will help the onsite team optimize the patient for safe transport and coordinate the transfer process for your team with the other Intermountain Health support services, including transfer center, patient placement, and medical transport.

Our specialists share what they know, walk your caregivers through recommended treatments, and ultimately help them feel confident in keeping higher-acuity patients in your facility – closer to home.

Intermountain has seen telehealth patients have outcomes equal to that of a patient receiving in-person care at one of our facilities. This is done through partnered implementation of and adherence to Intermountain’s best practice standards of care.

Telehealth technology expands the reach of an Intermountain specialist to multiple facilities where on-site caregivers are able to deliver in-person care with the oversight and experience of a specialty trained provider. Rethinking care in this way allows patients to stay closer to home, local facilities to provide a higher-level of care, and revenues to stay at the local facility.
