Advanced Practice Provider

Nurse Navigator

Your Nurse Navigator plays a key role in your journey.

A Nurse Navigator helps patients with kidney disease learn more about their condition. If you need dialysis, they will help you know what to expect.

Social Workers

Your Social Workers plays a key role in your journey.

Dealing with kidney failure and dialysis can be stressful and emotional. A social worker helps you deal with insurance, find support, and manage your diagnosis.
