What to expect

During your first visit with our team, you will meet with a provider, nurse coordinator, dietitian, and behavioral health professional for an evaluation of your unique needs. The team will discuss goals, follow-up tests, and your plan of care. Plan for this appointment to last at least one hour.

You will have regular follow-up appointments every 1-3 months as recommended by your provider. During follow-up appointments, you will meet with our multidisciplinary team of providers to review progress toward goals and revisit your individualized plan of care. The provider will order necessary laboratory tests and refer to additional specialists if needed. The provider may recommend additional meetings with your dietitian in between PeDWISE Program appointments.
