If you would like copies of your health records for yourself there are several options to obtain your records. Choose the option that best fits your needs.
The Intermountain Health Patient Portal
The Patient Portal is available for free as a personalized mobile app and web experience. The Patient Portal gives you access to a limited set of your health record at no cost to you. Those records include history and physicals, discharge summaries, lab results, image reports, ED notes and progress notes.
- If you are currently registered for the Patient Portal, sign in to access your records.
- If you are not registered for the Patient Portal, download the mobile app to get started.
Release to Self Authorization Form
You may also request access to your medical record by downloading a copy of the Patient Request to Access Medical Records Form (also available in Spanish), completing it and returning it via mail, email, fax or submit your form using the web submission form. Charges may apply for medical record copies or CD/DVD radiology images released. Medical records requests will be filled within 30 days of your request.
