Advance Care Planning (ACP) is thinking and talking about future healthcare decisions long before a sudden, emergent event, like a car accident happens and you are unable to make your own decisions. By having these conversations early, you can ensure your wishes are known and respected by documenting them in an advance directive. The process includes:
- Understanding possible future health choices
- Reflecting on these choices in light of the values and choices important to you
- Discussing your choices with those close to you and health professionals who care for you
- Making a plan for future healthcare situations, including who will make choices for you. This plan is written on an advance directive to be shared with your healthcare team, your loved ones and the person who will make decisions for you.
