Ambulatory clinical pharmacy team

Lutheran Hospital Ambulatory Clinical Pharmacy Services comprises a team of ambulatory clinical pharmacists and a pharmacy technician who provide care to patients referred to the service for blood thinner monitoring, diabetes, hypertension, heart failure, asthma, COPD, and other chronic disease states. This specialty-type service helps ensure patients are on guideline-directed therapy, achieving their disease state goals, and living healthier lives. Additionally, this team specializes in medication access, mitigating barriers to receiving the medications necessary for optimal care.
