Our Hospice Care services include:

  • Pain & Symptom Management: We ease discomfort and manage symptoms using medications, therapies, and interventions to ensure comfort.
  • Pharmacy Services: Medications for terminal illness and symptom control are delivered directly to your home. For medications that are not to manage your terminal illness you can use our pharmacy services.
  • Medical Equipment & Supplies: Essential equipment and supplies, such as hospital beds, wheelchairs, and oxygen, are provided to enhance comfort and quality of life.
  • Caregiver Respite & Support: We offer respite care and emotional support for caregivers, along with education and guidance to help them manage challenges.
  • Volunteer Support: Our volunteers offer companionship, practical help, and emotional support, contributing to your overall well-being.
  • Bereavement Support: We provide compassionate grief support, including counseling, support groups, and memorial services, to help families navigate loss.

When to choose Hospice Care in your home

Hospice in your home may be right for you or your loved one if:

  • You or your loved one has a terminal illness and is expected to live six months or less and would like to focus on comfort rather than aggressive treatments.
  • You and your family decide that managing pain and symptoms at home is the best way to ensure comfort and avoid frequent hospital visits, allowing you or your loved one to remain surrounded by family and familiar surroundings.
  • There’s a strong network of family and friends ready to assist with daily care and coordinate with hospice professionals.

Frequently asked questions regarding Hospice Care.

Hospice is a compassionate approach to end-of-life care. The goal of Hospice Care is to help people with serious illnesses live life fully, maintain their dignity, and keep personal control over their lives. Hospice Care provides end-of-life care, including emotional and spiritual support to patients and their families facing life-limiting illness in homes, hospitals, and nursing homes. Hospice Care is available to anyone with an illness whose life expectancy is measured in months rather than years.

Medicare, Medicaid, and most commercial insurance plans and HMOs cover hospice services. Patients without adequate insurance or limited resources can apply for special financial assistance supported by donations to Intermountain Homecare.

Unique physical, emotional, spiritual, social, and financial needs occur during each phase of a terminal illness. Hospice Care provides experienced and skilled specialists who understand the help and support needed during this difficult time. The hospice team works with the attending physician and family to develop a plan of care that meets the patient and family's needs and wishes.

Hospice Care serves individuals of any age and with any terminal illness, including cancer, heart disease, kidney disease, neurological conditions, ALS (Lou Gehrig's Disease), Alzheimer's disease, cirrhosis, leukemia, and many others.

At any time during a life-limiting illness, discussing all of a patient's care options, including Hospice Care, is appropriate. When curative treatments are no longer effective, Hospice Care is reasonable. Federal law requires a doctor to certify the patient's life expectancy is six months or less.

Sometimes, but not always. It may be up to the patient or family member to start the discussion about Hospice Care. You are free to contact us to help determine if Hospice Care may be appropriate and whether to discuss it with your doctor.

There are two ways. Your doctor or hospital case manager, or you may call us directly. If you or someone from your family calls, we will contact your doctor to discuss the illness and verify that Hospice Care is medically appropriate. The patient is admitted to Intermountain Homecare Hospice Services by signing a consent form electing Hospice Care.

Yes. Hospice Care nurses will work with the patient's doctor on all aspects of care. The local Intermountain Homecare Hospice Medical Director is available for consultation with the patient's doctor.

The Hospice Care nurse is available 24 hours a day by phone to assist with questions or emergencies and can make a home visit if needed. Hospice Care pharmacies are available off-hours to ensure that changing medication needs can be met anytime.

Bereavement refers to the process of grieving and mourning and is associated with a deep sense of loss and sadness. Bereavement services are provided to surviving families for at least one year after the death of a patient. Bereavement services include ongoing contact with family members/caregivers, bereavement support groups, written educational materials for coping with grief that are mailed, and referral to outside counseling as needed for those experiencing complicated grief.

Intermountain Homecare provides the following bereavement support services:

  • Follow-up supportive services to bereaved family members for twelve months.
  • Mailing of educational materials and intermittent follow-up contact and support.
  • Specially trained clinicians facilitate bereavement support groups.
  • Support groups are offered free of charge and are open to the general public.
  • Referrals to community services.
