How we collaborate

To inform the work of the Community Health team, Intermountain Health conducts a Community Health Needs Assessment (CHNA) and Implementation Strategies for its communities in Colorado, Montana, Nevada, southeast Idaho, and Utah. We invest time listening to our communities to ensure our initiatives address the needs you, our communities, identify.
