We listen

We take a collaborative approach to community health improvement by routinely conducting Community Health Needs Assessments (CHNAs) in our service areas. During the CHNA process we listen to community leaders and members with diverse backgrounds, voices, and experiences to understand their unique health needs and the community-based assets.

Through the CHNA, we identify Significant Health Needs that we prioritize for the next three years.

We improve health

To address Significant Health Needs, we develop an Implementation Strategy in collaboration with our communities that focuses on data-driven needs and solutions.

We believe everyone deserves equitable opportunities for health. We are working with our communities to access healthcare, including removing barriers in language, cultural appropriateness, insurance or other coverage enrollment, and navigation of complex systems of care.

We take a proactive approach to health including strategies to address avoidable chronic illnesses, the leading cause of death in adults, and prevent injuries, the leading cause of death in children.

We address social drivers of health such as housing, food, and economic stability by building and supporting local programs and collaborations.

We also leverage our system assets as an anchor institution to improve the welfare and economic well-being of our communities.

Children have health needs and disparities that may differ from adults. We look at causes of morbidity and mortality in children and to develop strategies focused on their life-long health.

At Intermountain Health, volunteering is more than a moment of service—it’s part of how we care for our communities. Our Community Health strategy is based on what we learn from local Community Health Needs Assessments and supports our mission to help people live the healthiest lives possible.

Home is where health, stability and belonging begin. This belief is part of Intermountain Health’s nonprofit mission and drives our Place Based Investing (PBI) program. Through low-cost loans and flexible financing, we support community projects that strengthen the places where people live and work.

The Medically Vulnerable Program is a collaboration between Fourth Street Clinic in Salt Lake City, Utah, and the Intermountain Community Care Foundation. The program provided Mary with safe housing, coordinated medical care, and emotional support.

As summer approaches, ensuring children's safety around water is a top priority. Through collaborations with local health and community agencies, Intermountain Health is donating life jackets to help people enjoy the water and be safe.

Intermountain is working to reduce opioid deaths, along with addressing the stigma and barriers that often prevent people from seeking help. This work aligns with the significant needs identified in our Community Health Needs Assessments as a key issue faced in our communities.

Improving mental well-being is a significant health priority at Intermountain Health. Our Community Health and Children’s Health teams are taking on initiatives to support this focus.

Intermountain’s Community Health team was recognized nationally for their work that drastically reduced repeat offender bookings in the Sanpete County Jail in rural central Utah.

St. Mary’s Regional Hospital in Grand Junction, Colorado, hosted a Poverty Immersion Experience in conjunction with United Way of Mesa County and the City of Grand Junction’s Housing Department.
