Support for Patients and Families

If patients or parents are interested in participating in a support group, please contact a social worker by calling 801-662-6354 (Phone - SLC), 801-662-6339 (Phone - SLC), or  385-531-3527 (Phone - Lehi).

Our clinical social workers meet with patients and families during clinic provider visits to help address mental Health needs, psychosocial well-being, and provide resources if needed.

Each November we host an awards night for clinic patients who have had diabetes for 10 years or more. The Lilly Diabetes Journey Awards honor individuals who have managed their diabetes for 10, 25, 50, and even 75 years.

As our patients are under 18, we are unable to nominate them ourselves. If you are interested in participating, please talk to one of the social workers for more information.
