What is Personalized Primary Care?

Personalized Primary Care is our patient-centered model of healthcare delivery implemented by the Intermountain Medical Group and Roy Clinic. This model is guided by Intermountain Healthcare’s Mission, Vision, and Values and is based on the National Committee on Quality Assurance 2011 Patient-Centered Medical Home Standards.

How does it work?

Our team works together to help patients get healthy, stay healthy and get the right care. We focus on making sure every patient is provided with the right services at the right time and in the right place. All team member works hard to provide each patient care for their individual needs.

Who is involved?

The Roy Clinic team may include a Physicians, an Advanced Practice Clinician (NP or PA), a Care Manager, a Health Advocate, Mental Health Partners, and other clinical and office staff. Our physicians specialize in a variety of medical treatments for each patient including suboxone treatment, chronic illness diagnosis, wellness checkups, and more.
