Heart Center Nurse Navigation and Outreach Program

We know that traveling for healthcare is challenging and, at times, overwhelming. To help make the process easier, the Heart Center at Primary Children’s Hospital has a nurse navigation team to assist families having cardiac interventions at Primary Children’s Hospital.

The nurse navigators with the Heart Center work with both local cardiologists and referring cardiologists to coordinate care for children coming to Primary Children’s Hospital for cardiac cath, electrophysiology, or surgical procedures. Additionally, the nurse navigation outreach team can coordinate visits to the cardiology subspeciality clinics with imaging and diagnostic testing.

  • The nurse navigators serve as a single point of contact for families to provide education and support from the time the referral is received through discharge
  • The nurse navigators ensure that necessary testing and diagnostics are completed prior to any procedures or clinic visits
  • The nurse navigators assist with communication back to referring providers from time of referral until after discharge

Your Nurse Navigators are Jamie Park, RN; Murphy Dwyer, RN, IBCLC; and Corinne Ballard, RN, CIC.

Outreach Team

Jamie Park, RN – Senior Nurse Navigator for Outreach

Jamie started taking care of children with congenital heart disease in 1999. Jamie also has a background in early intervention and neurodevelopmental care post ICU stay. Jamie assists families who are traveling for surgery. When not at the hospital, Jamie enjoys camping just about anywhere and reading.

Corinne Ballard, RN

Corinne has been working at Primary Children’s hospital since 2003 gaining a variety of skills in a couple different roles. She spent eight years caring for patients in the Cardiac and Pediatric Intensive Care Unit which produced a passion for patients with cardiac disease. In her free time Corinne loves to be outside and spend time with her dog. Corinne assists families who are traveling for procedures in the CATH and electrophysiology lab.

Murphy Dwyer, BSN, RN, IBCLC

Murphy has worked in women’s and children’s health for the last nine years. This includes time as a NICU nurse, lactation consultant, childbirth educator, and pediatric critical care case manager. Murphy’s goals for this role are to make the heart surgery process as straightforward as possible by being a single point of contact that helps families navigate the healthcare system, from referral to surgery

Tania Nay - Outreach Administrative Coordinator

At the Heart Center, we also have an Outreach Administrative Coordinator, Tania Nay, to facilitate the flow of all surgical and cardiac interventional referrals. Tania has been with Intermountain Health since 2009. She previously worked in the Primary Children’s Surgical Services Department as the lead Administrative Assistant and patient care tech for 10 years. She is a single mother to one daughter and loves helping take care of the needs of our patient population within the Heart Center.

We use Nuance Power Share as our web-based platform to send and receive imaging studies.
