House Calls brings coordinated, compassionate care to adults with complex medical needs—right at home—so you can stay safe, supported, and connected to the care you need.

Services are available across much of Utah. Some remote areas are excluded due to travel distance and service limitations.

House Calls is designed to help you overcome barriers that make traditional clinic visits difficult. Our team adapts services to your health, home environment, and daily challenges.

  • In‑home medical visits - Providers come to your home to understand your needs, evaluate your health, and offer personalized recommendations.
  • Care planning tailored to your situation - We work with you and your family to develop a plan that supports your health, mobility, and overall well‑being. 
  • Coordinated medical support - The House Calls team serves as an extension of your primary care clinic—helping manage medications, follow‑ups, and chronic conditions.
  • In‑home services (labs, imaging, and more) - Some diagnostic services can be completed in your home, helping reduce the burden of travel. 
  • Help accessing community resources - We connect you with medical, mental‑health, financial, transportation, or home‑support resources in your community. 
  • Ongoing follow‑up and support - We check in regularly to ensure your care plan is working and adjust as your needs change.

Adults who have recently been hospitalized or who have complex or chronic medical needs that make it difficult to attend clinic appointments may qualify. Your primary care provider can help determine whether the program is a good fit.
No. There is no additional cost to enroll in the program. Standard insurance coverage still applies, and your team can help answer questions about financial resources if needed.
Your House Calls team partners with your primary care provider throughout your enrollment. Their goal is to help stabilize your care and return you to regular clinic visits when you’re ready.
Depending on your needs, services may include wellness checks, medication support, chronic‑condition monitoring, lab work, imaging, and coordination of additional care services.
Yes. While on our program, we can connect you to resources for assistance with mental-health services, community programs, transportation resources.
