Check in at the Park City Specialty Clinic registration desk and a sleep technician will be paged.

Sleep studies are conducted in the evenings, Sunday through Saturday from 8:00 PM to 7:00 AM.

Please fill out the form below to request an appointment with the Sleep Disorders Center. We will contact you within 24 hours or the next business day.

If you have scheduled an appointment and need to cancel, you will need to call in. Appointments must be canceled over the phone.

This form should not be used for urgent care matters or potentially life threatening conditions. In those situations, please contact 911 or head to the nearest InstaCare or emergency room.
