Need your COVID-19 vaccine card for traveling? Get a replacement from Employee Health

If you’ve lost or misplaced your COVID-19 vaccination card, you can get a replacement card from Employee Health. This applies only to caregivers who received a COVID-19 vaccine from an Employee Health sponsored clinic in December, January, February, or March. To get a replacement card, contact your local Employee Health nurse with your name, employee ID, and date range as to when you received the vaccine. Employee Health is unable to provide new vaccine cards if you were immunized at a community clinic or pharmacy.

Remember, before you travel this summer, check with your destination to find out if they have specific requirements for testing, masking, or proof of immunization. Infection Prevention director Cherie Frame recently ran into a problem by not having the correct proof of immunization while traveling.

“My travel destination wouldn’t accept the photo I had of my vaccine card. They wanted the actual card as proof,” says Cherie. “I had to be tested before they would allow me into the county of the state I visited.”

Intermountain strongly encourages caregivers to get the COVID-19 vaccine, but it isn’t mandated. If you haven’t received your COVID-19 vaccine, you can schedule an appointment at intermountain.com/covidvaccine.
