All caregivers without a documented exemption need to get their flu shot by November 30. There are no Intermountain caregiver clinics planned for this week, but there are some clinics planned for next week (and you can also get a COVID shot while you’re there). If you don’t get your flu shot at an Employee Health caregiver flu clinic—including if you get it from an Intermountain provider—you’ll need to submit documentation to Employee Health by next Tuesday, November 30.
To provide documentation of your flu shot, send an image or pdf of your EOB or paperwork prepared when you get your shot to EmployeeHealthFluShots@imail.org. The documentation needs to include the following information:
- Your name
- Your birthdate
- The name of the location where you got your shot
- The date of your vaccination
- The type of shot you got
- The name or signature of the person who administered your shot (if available)
