Thank you to those who've already met the COVID-19 vaccine requirement

Thanks to the thousands of caregivers who’ve already provided consent and submitted documentation of your COVID-19 vaccine status or have been approved for an exemption. Thank you for helping us be compliant with Centers for Medicare & Medicaid Services (CMS) and federal contractor regulations so we can continue to serve hundreds of thousands of Medicare and Medicaid patients. If you haven’t yet provided consent and submitted documentation, there are only 12 more days to meet the COVID-19 vaccine requirement by the deadline, which is Sunday, December 5. CMS guidelines say caregivers must receive the first dose of Pfizer or Moderna or the single dose of Johnson and Johnson, or have an approved exemption in place by this date.

There’s still time to meet the requirement. Here’s how to follow up:

  • Get vaccinated. Schedule an appointment to get your COVID-19 vaccine at a caregiver vaccine clinic or visit a community site and upload your documentation before December 5, 2021. While there are no vaccine clinics happening this week because of Thanksgiving, there are lots of appointments available next week.
  • Give consent and upload vaccine documentation. Click this link to give consent for Intermountain to access your COVID-19 vaccine record and upload documentation if needed. This step must be completed even if you received your vaccine from Intermountain. The link only works when connected to the Intermountain network (e.g. on a facility computer or through PingID/PulseSecure) through Google Chrome or Microsoft Edge.

If you don’t want to give consent for Intermountain to access your vaccine record digitally, you can click this link, click the “Immunizations” button, and then “Add COVID Vaccination” to upload your documentation, or email your documentation to ehservices@imail.org. By doing this, Employee Health will only use the record(s) you upload and share.

  • The deadline to apply for a medical or religious exemption has passed. Exemption notifications—both approvals and denials—are being sent daily. Emails are sent to the email the caregiver provided on their application—personal or professional. The email comes from Immunization Exemptions <noreply@qemailserver.com>.

The best way to check if you’ve already met the requirement is to visit the consent link to validate that your information has populated, or ask your leader to check their compliance report.
