Time is running out to meet Intermountain's COVID-19 vaccine requirement

In accordance with guidelines set by the Centers for Medicare and Medicaid Services (CMS), all caregivers must by this Sunday, December 5, receive at least one dose of Pfizer or Moderna COVID-19 vaccine, or the single dose of Johnson & Johnson, and submit consent and proof of vaccination, or have an approved exemption on file. More than 40,000 caregivers—nearly 95% of our workforce—have met the vaccine requirement so far.

The best way to check if you’ve already met the requirement is to visit the consent link to validate that your information has populated, or ask your leader to check their compliance report. If you’re receiving compliance reminder emails, you haven’t yet met the requirement.

If you haven’t already, take action by December 5:

  • Give consent and upload vaccine documentation. Click this link to give consent for Intermountain to access your COVID-19 vaccine record and upload documentation if needed. This step must be completed even if you received your vaccine from Intermountain. The link only works when connected to the Intermountain network (e.g. on a facility computer or through PingID/PulseSecure) through Google Chrome or Microsoft Edge.

If you don’t want to give consent for Intermountain to access your vaccine record digitally, you can click this link, click the “Immunizations” button, and then “Add COVID Vaccination” to upload your documentation, or email your documentation to ehservices@imail.org. By doing this, Employee Health will only use the record(s) that you upload and share.

  • Get vaccinated. Schedule an appointment to get your COVID-19 vaccine at a caregiver vaccine clinic. Appointments are available now. You can also visit a community site and upload your documentation by clicking this link by December 5, 2021.
  • If you’ve emailed documentation to Employee Health, please be patient. They’re working as quickly as possible to validate and input vaccine documentation by December 5. Please don’t call Employee Health to check if you’re compliant.
  • Remember, you must receive the second dose of Pfizer or Moderna vaccine (if you received a Pfizer or Moderna vaccine as your first dose) by January 4, 2022. This is the second major deadline caregivers must meet.
