There’s still time to nominate a fellow a caregiver, leader, LiVe Well Champion, a team of caregivers, or family (an employed caregiver and their family members) for a LiVe Well Hero award. The nomination deadline has been extended to Sunday, February 13. A LiVe Well Hero is someone who has inspired you and others through their efforts to improve their well-being in any of the five dimensions of LiVe Well in 2021. LiVe Well Heroes move us closer to fulfilling Intermountain’s mission through their choices and actions.
To nominate a LiVe Well Hero:
- Visit the Intermountain Recognition Portal: www.intermountainrecognition.com. You’ll need to be on an Intermountain computer or logged onto Secure Access via PingID for the site to work.
- Click on the “LiVe Well Heroes” dropdown, located on the top bar of home page, next to your name.
- Complete the nomination form.
- Select “yes” that you want to submit your submission.
- Your nomination will be forwarded to the LiVe Well team.
