This Sunday, February 13, is the first major deadline for the COVID-19 vaccine requirement according to federal regulations from the Centers for Medicare and Medicaid Services (CMS). You must receive at least one dose of Pfizer or Moderna, or the single dose of Johnson & Johnson, and submit consent and proof of vaccination, or have an approved exemption on file by this date.

The best way to check if you’re compliant is to visit the consent link to validate that your information is there, or ask your leader to check their compliance report. If you’ve been receiving compliance reminder emails from Employee Health, you haven’t yet met the requirement. 


There’s still time to meet the February 13 deadline:


  • Give consent and upload vaccine documentation. Click to give consent for Intermountain to access your COVID-19 vaccine record and upload documentation if needed. The link only works when connected to the Intermountain network (e.g., on a facility computer or through PingID/PulseSecure) through Google Chrome or Microsoft Edge.

If you don’t want to give consent for Intermountain to access your vaccine record digitally, you can click on this link, click the “Immunizations” button, and then click “Add COVID Vaccination” to upload your documentation. You can also email documentation to ehservices@imail.org.

  • Get vaccinated. Schedule an appointment at a location near you on vaccines.gov and submit your vaccine documentation by February 13.
  • Apply for a medical or religious exemption. Applications for medical and religious exemptions are still being accepted and reviewed. If you’re interested in applying, please do it as soon as possible so the committees have adequate time to review and approve (or deny.) Emails about your exemption status come from the sender Immunization Exemptions at noreply@qemailserver.com. It will have a red “External Sender” header on it. Please check both your spam and junk folders for this email before reaching out to Ask HR. Do not submit a duplicate request, unless otherwise directed.

If you don’t hear about your exemption status (approved or denied) by February 13, you’ll be taken off the work schedule until you hear otherwise.

  • If you emailed documentation to Employee Health, please be patient. Employee Health and Human Resources will be validating and inputting information through the weekend. Please do not call Employee Health to check if you are compliant.

As a reminder, caregivers who’ve chosen a two-dose Pfizer or Moderna COVID-19 vaccine must receive the second dose by Tuesday, March 15. This is the second major deadline.

Caregivers who don’t meet the COVID-19 vaccine requirement by the deadlines will be subject to corrective action, including suspension.
