New My Health+ tool makes advance care planning more accessible

Vynca offers an interactive tool on the My Health+ app.

My Health+ now features a new tool that allows users to more easily complete the advance care planning process online. Created in partnership with Vynca, the tool is available on computers and smart phones.

Under development since last summer, the Vynca tool makes it easier to create an advance directive, which is a legal document that identifies who can speak for you in a health crisis if you can’t speak for yourself. In addition, advance directives clarify what kind of healthcare you want to receive in an emergency.

Advance directive forms are unique by state. The tool is now only available in Utah, although other states will be added in the future.

Once an advance directive is completed using Vynca, a copy automatically goes to a patient’s electronic medical record. Users have the option to download and save a copy for personal use as well. If someone doesn’t already have a medical record in iCentra, the app creates one when they set up a new My Health+ account.

The Vynca tool is the first of many interactive tools on the My Health+ app found under the “Health Tools” link. A behavioral health tool called “myStrength” is set to launch in the coming weeks, and a series of pre-diabetes educational videos will follow. My Health+ is also surveying users to identify other health topics of interest with the ultimate goal of providing digital tools that help people live the healthiest lives possible.
