Advance Directives

Rose Canyon Clinic, and Intermountain Healthcare as a whole, would like to encourage everyone to complete an Advance Directive. It’s a simple way for you to express what your individual needs and wishes are when it comes to your health care. This is particularly important during an emergency or for a time when you might not be able to talk with your doctor to make medical decisions. We would suggest that anyone over the age of 18 should take the time to think about and write down their heath care wishes.
 
An Advance Directive is just a form you fill out that lets physicians know your personal preferences for treatment. It also gives you the opportunity to designate a specific person to make treatment and care decisions on your behalf in the event that you are not able to make decisions for yourself. Of course, you can always make changes to your Advance Directive form at any time, and you should make changes when circumstances change.
