We listen
We take a collaborative approach to improving health equity by routinely conducting Community Health Needs Assessments (CHNAs) in our service areas. During the CHNA process we listen to community leaders and members with diverse backgrounds, voices, and experiences to understand their unique health needs.
Through the CHNA, we identify Significant Health Needs that we prioritize for the next three years cycle. Additionally, we prioritize Sustaining Health Needs through morbidity and mortality data on children and youth, which may differ from the adult population.
