To qualify for this service, you must be 18 years of age or older and have a history of recurrent cold sores. You will not qualify for this service if you:

• Are pregnant or trying to become pregnant
• Have HIV, other immunocompromising conditions, or taking immunocompromising medications
• Have lesions on other parts of your body besides mouth/lips

To get started, you will need:

• Your prescription insurance information if you have it (not required)
• Current list of medications
• Current medical conditions

The cost of the service is a $20 consultation fee plus the cost of the medication after any insurance coverage. Cost of medication will vary depending on selected products and insurance coverage. If you do not qualify, you will not be charged for the service.

Fill out another form and indicate that you received treatment previously from the On-Demand Service and a pharmacist will call you and assist you. For any severe adverse reactions, please seek care at your nearest urgent care or emergency department

Complete the online intake form so a licensed pharmacist can determine the right treatment for you. A pharmacist will call you within 24–48 hours to discuss treatment options.

We’ll make up to two phone calls and one email if we can’t reach you right away by phone. Usually, we cannot provide a prescription without speaking with you first.

Refunds are not given if we cannot reach you via phone or email. Bank payments cannot be refunded.

If too much time passes after you send your form and we can’t reach you, you may need to fill out a new one so we have your updated health information.
