Learn more about what to expect with this service
- You are above 18 years of age
- Your body mass index (BMI) is over 30 for starting new treatment or over 25 for continuing treatment. BMI will be calculated on intake form
- You have had a visit with your primary care provider in the last 12 months or will have a visit in the next 12 months. Service may be discontinued if these requirements at not met.
- About 10-15 minutes to answer questions about your health history on the intake form
- Medications you have tried in the past
- Prescription insurance information if a medication is prescribed
- Evaluation from one of our clinical pharmacists to assist in prescribing injectable weight loss medications also known as GLP-1
- Counseling regarding correct use and potential side effects
- If eligible, you will receive up to 19 months of commercially available injectable GLP-1 products. This service is provided for up to19 months OR ≤BMI 25, which ever occurs first. You will need to follow up with your primary care provider if you would like to continue treatment.
- Please note that we do not offer taper down treatment once appropriate weight has been reached. We also do not offer microdosing. These services are best provided by your primary care provider.
Complete the online intake form so a licensed pharmacist can determine the right treatment for you. A pharmacist will call you within 24–48 hours to discuss treatment options.
We’ll make up to two phone calls and one email if we can’t reach you right away by phone. Usually, we cannot provide a prescription without speaking with you first.
Refunds are not given if we cannot reach you via phone or email. Bank payments cannot be refunded.
If too much time passes after you send your form and we can’t reach you, you may need to fill out a new one so we have your updated health information.
