Once you complete the intake form, a clinical pharmacist will contact you and determine qualification. This service is a great option if:
- You are 18 years old or older
- You don’t have cervical or uterine cancer
- You are not pregnant or think that you may be pregnant
- You do not have fever, nausea, vomiting, vaginal bleeding, or pelvic pain
- You have not had more than 2 vaginal yeast infections this year
- If you do not have new sexual partners or think it’s possible you have a Sexually Transmitted Infection (STI)
- You are not having pain, burning, or bleeding with urination

- The cost of the service is $20 consultation fee plus the cost of prescription treatment (if medication is ordered during the visit) after any insurance coverage. Some insurance companies will also cover the $20 consultation fee. If you do not qualify, you will not be charged for the service.
- All patients with suspicion of vaginal yeast infection are recommended to visit their PCP, OB-GYN, or urgent care to test for presence of concomitant infections such as mycoplasma, bacterial vaginosis, or STIs as these can occur at the same time as yeast infections or have similar symptoms to a yeast infection.
- You can expect to be contacted by the clinical pharmacist within a few hours (between 8am and 5pm MST) following submission of the intake form. Generally, the medication will be sent to your local pharmacy. The clinical pharmacist will work with you to identify a different arrangement if a more rapid turnaround is needed.
- You will be instructed on using the medication correctly, educated on when you will feel better, and told what to do if you don’t feel better after any treatment is prescribed.
Complete the online intake form so a licensed pharmacist can determine the right treatment for you. A pharmacist will call you within 24–48 hours to discuss treatment options.
We’ll make up to two phone calls and one email if we can’t reach you right away by phone. Usually, we cannot provide a prescription without speaking with you first.
Refunds are not given if we cannot reach you via phone or email. Bank payments cannot be refunded.
If too much time passes after you send your form and we can’t reach you, you may need to fill out a new one so we have your updated health information.
