The Leapfrog Hospital Safety Grade uses national performance measures from the Centers for Medicare & Medicaid Services (CMS), the Leapfrog Hospital Survey, the Agency for Healthcare Research and Quality (AHRQ), the Centers for Disease Control and Prevention (CDC), and the American Hospital Association's Annual Survey and Health Information Technology Supplement.
CMS created the Five-Star Quality Rating System to help consumers, their families, and caregivers compare healthcare facilities more easily and to help identify areas about which you may want to ask questions. Facilities with 5 stars are considered to have much above average quality and facilities with 1 star are considered to have quality much below average.
