Partners will gain access to Intermountain’s Care Process Models, build collaborative relationships with and learn from best-in-class specialists, and gain the benefits of belonging to a large healthcare system while maintaining independence.
Our goal is to keep patients in their communities whenever possible. If, however, a transfer is needed, partnership with Connect Care Pro also includes a suite of Provider Support Services. Clinical Coordination Teams, including intensivists and hospitalists, consult with onsite providers on the need for a transfer, optimize a patient for safe transport, and coordinate with the other Connect Care Pro Provider Support Services, including Transfer Center, Patient Placement, and Life Flight.
We work to
- Support and collaborate with local care teams to safely care for patients in their local hospital and avoid unnecessary transfers.
- Streamline the transfer process for patients who need higher level care.
A partnership with Connect Care Pro provides support and collaboration for local providers in clinics and hospitals and keeps patients as close to home as possible, improving the health and vitality of communities throughout the Intermountain West.
