The LiVe Well Center Assessment Plus include all of the components from the LiVe Well Center Assessment—an evaluation of your nutrition, physical activity, stress, sleep, relationships, and emotional health with a customized Wellness Profile and game plan for improving your well being. A lifestyle medicine consult at one of our LiVe Well Centers will create a personalized and collaborative plan with you by working in partnership with you on the following;

• Personal Health Goal Planning

Intermountain Arthritis Program: Insurance (Initial Physical Fitness Screening:$50)
Get help managing arthritis pain with nutrition education focused on an anti-inflammatory diet, specialized fitness classes, and stress management support. This program is excellent for those with chronic arthritis pain who may or may not be considering joint replacement or lower back surgery, as well as for those who have already had surgery and are looking to improve their quality of life. Each year, more than a third of adults 65 and older experience a fall, often resulting in injuries and hospitalization. Stepping On is a free seven-week program designed to help older adults reduce their risk of falls. Stepping On teaches tips for improving balance, helping you build confidence and maintain your independence. The Stepping On program has been proven to reduce falls by 30% by addressing balance issues, medications that can cause dizziness, and vision impairment.
A free six-week workshop series for those living with chronic conditions such as diabetes, heart disease, arthritis, asthma, chronic pain, depression, fatigue, and more. A licensed social worker and other professionals will give you tips and advice to help you live life to the fullest despite your condition.
Living Well with Chronic Pain Class: Free (6 weeks)
This program introduces new ways of managing the day to day symptoms of long-term/chronic pain. Over the course of the 6-week program you will learn ways to structure your day to get the most out of each moment, tips and tricks for tackling projects, ways to improve physical activity and more. Learn ways to better communicate with loved ones about your pain, ways to work with your healthcare team and connect with others who are also living with long-term/chronic pain.
