Primary Children's Diabetes Clinic and diabetes self-management education program provides ongoing comprehensive interdisciplinary education and specialized care to children and adolescents with diabetes type 1 or type 2, and their families.
Our diabetes self-management education program has been recognized by the American Diabetes Association (ADA) for meeting national standards for providing diabetes education to patients and their families.

After diagnosis, your child will be seen in the diabetes clinic. Here's what to expect.
Hospital Follow-up Class
Following diagnosis, you will attend the new onset class that will be 2-3 hours. During the class, you will receive more in-depth information in regards to diabetes and meet individually with a diabetes educator and other team members.
Initial Clinic Visit
You will attend your first clinic visit about 4 - 6 weeks following diagnosis. During this visit you will meet with a provider, social worker and a certified diabetes educator. This initial visit will take longer than most clinic visits, plan to be in clinic for 2 - 3 hours.
Follow Up
Patients will have regular follow-up appointments as recommended by a provider, usually appointments are every 3 months. These appointments usually last about 60 minutes.
Schools require Diabetes Medical Management Orders (DMMO) from your provider to care for your child’s diabetes at school. This form can be filled out by our office and sent to you in the Intermountain Patient Portal at the beginning of the school year. Please contact our office through the Intermountain Patient Portal or at diabetes@imail.org when you need these orders.

Each November we host an awards night for clinic patients who have had diabetes for 10 years or more. The Lilly Diabetes Journey Awards honor individuals who have managed their diabetes for 10, 25, 50, and even 75 years.
As our patients are under 18, we are unable to nominate them ourselves. If you are interested in participating, please talk to one of the social workers for more information.
